Medical Cannabis Patient Program
IDPH will accept petitions on forms and in a manner prescribed by IDPH. The forms and instructions are available here. As of June 22, 2021, the Medical Cannabis Patient Program is no longer mailing medical cannabis registration cards to current patients and new program applicants.
